CASE STUDY:


Failure description:
Loco no 11522 W/W train no ,Train arrived BPQ station at1855hrs. Loco fuelling done at 1900hrs .While starting loco got automatically shutdown. Loco was recranked and started at 1935hrs or 15”late start.

Examination details:
After receiving the message locomotive data was analysed through REMLOT. Train was stopped at 18.49hrs. loco no 11522 was rear loco and it was fitted with APU .Hence , after 12 “ aprx the APU engine was started and main engine got shut down as per normal working of APU locomotives. Since LP was not aware of working of APU in 11522 , he tried to crank the loco in random manner. Finally LP switched off all the breakers and re cranked the locomotive. This caused time loss. Time event of activities are as follows, 
1. At 18.49 hrs –Train stopped 
2. At 1901 hrs – APU started – Main engine shut down. 
3. At 19.02 hrs – APU stopped – Due to RH FOR/FEV and main engine cranked 
4. At 19.16 hrs – APU started – Main engine shut down Then LP noticed rear loco shut down 
5. At 1934 Hrs – Power OFF – breaker was switched OFF 
6. At 19.35 Hrs - Power ON – Breaker was switched ON 
7. At 1937 Hrs – LP recranked the loco – Further train worked.

Cause description:
LP tried to crank the loco without knowledge of APU working.
